| Michael Carter is an expert witness with expertise in the field of fetal monitoring. His particular expertise is in the technical analysis of the fetal heart rate (FHR) and maternal uterine contraction records made during pregnancy and labour. The recording produced by a fetal monitor is known as the cardiotocogram (CTG). An example is shown on the top banner of this page. Generally, fetal monitors will accurately record the fetal heart rate and midwives and obstetricians are able to interpret the recording. However, in a very small number of cases a fetal monitor can produce erroneous information. An in-depth knowledge of the characteristics of the particular model of fetal monitor is necessary to determine whether the CTG is a true record of the FHR or if the CTG has been degraded by the presence of false tracings. There are a number of recurring problems that may be encountered when reviewing a CTG. The most common are related to the timing of the CTG, verification of the true FHR from equipment-induced interference, inadequate signal quality caused by poor application of the transducer and the inadvertent monitoring of the maternal pulse instead of the fetal heart rate. I have a specialist knowledge of the technologies used in fetal monitors and how it affects the appearance of the recording. My role in the interpretation of the recordings is essentially in ascertaining whether they are 'true' recordings. I can help the Court by saying what are artifacts and what are true records of the fetal heart rate and uterine activity and I have a specialist knowledge of what a monitor will produce in certain situations. How to react to a given recording is a clinical decision which is taken by a doctor or midwife, not by me. They take into account a number of other aspects of clinical information.
